If you experienced a qualified midyear change event and want to make midyear changes to your current year (2016) benefits or dependents, download the midyear change form and submit it to your agency payroll or university benefits office.
PEBB is mailing all members of our self-insured plans - PEBB Statewide and Providence Choice - SSNs of their covered dependents. The Affordable Care Act (ACA) requires that we ask for and provide this information to the IRS. The purpose is to match up provisions of the ACA with employer-provided health care benefits. The mailing gives instructions on providing the information through a secure online system.
