PEBB designed this site for members of its self-insured health plans (Moda Synergy, Providence Statewide and Providence Choice)
to provide their and their dependents Social Security Numbers (SSNs) - privately and in a secure online environment.
We are required to provide this information to the Internal Revenue Service (IRS) as part of the Affordable Care Act.

Before you begin, please have the following information at hand:
  • The names of your dependents covered in your PEBB medical plan.
  • The ages of your dependents covered in your PEBB medical plan.
  • The SSNs of your dependents covered in your PEBB medical plan.
  • The letter PEBB mailed containing your user name and password.
To log in, enter:
  • User ID as shown in the letter
  • Password as shown in the letter
    • If you received a password which contains the following characters 1, l, 0, then follow these guidelines:
      • 1 is the number one
      • l is always a lower-case (small) letter 'L'
      • 0 is always zero
      • There are no vowels used in the password
